In order to perform surgery on the lateral cheekbones through an intraoral incision, damage to the facial maintenance ligament located at the 45-degree cheekbone cannot be avoided due to the human facial structure. However, if the surgery is performed only through an incision in front of the ear, excluding the incision inside the mouth, will there be no sagging cheeks? In theory, it may be judged as such, but no matter how skilled a doctor is, if he or she performs surgery using only an incision in front of the ear, the surgery will be performed without being able to see the bone with the naked eye. Bone surgery will only be performed with the doctor's pressure. We all know that human movement is very dependent on vision.
Therefore, if you are going to have lateral cheekbone surgery, perform the surgery using an incision in front of the ear and an incision inside the mouth. However, the incision inside the mouth should be minimally dissected just enough to check the bone. If you over-excise with the expectation of a large effect, you may face side effects due to non-adhesion of the facial retaining ligaments. Since sagging of the remaining skin occurs according to the balloon theory, which is a slightly different problem, I think it is better to perform surgery with the goal of straightening only the facial lines from the front.
