When should I start taking hot packs?

I had my silicone + ear cartilage removed and had the tape removed today.. It's totally avatarㅋㅋ It looks funny. ㅋㅋ But. I feel relieved.. It's still swollen.. A little bruised.. Not much.. Today is my second day.. I'm getting the stitches removed next Tuesday. But. Hot compress... My nose is a bit stiff.. Currently.. It feels tingly.. When do you think I should apply the hot compress? Because it's still swollen... They told me to do a cold compress. And massage technique.. Please tell me.. I don't really understand even after reading it.. They told me to hold the tip of my nose and bend it with the thumb of the other hand.. And massage the bridge of my nose... Please explain in detail. But. You massage it when the scar forms and it gets hard, right?
