I'm in doubt about the pimple on my columella that I've had for 3 years.

It's a bit disgusting, but there's a picture.
 It's been 3-4 years since I had the surgery, and
 the first time I encountered that pimple was a month ago? I thought it would go away like a pimple, but suddenly it became bothersome.
 
I was thinking about squeezing it on my own, but I thought I shouldn't touch the location or area carelessly... I went to the ENT and asked if I touch my nose often, so I was given an ointment to just apply.
 But I watched something on YouTube recently and it said it could be cartilage??? I
 
would like to ask if anyone has ever had cartilage removal orthodontic treatment. Or maybe that's acne...
