thick skinned nose

I 'm looking for a hospital for rhinoplasty after looking at reviews.
 I have pretty thick skin (for example, no matter how many In Mode and Shurink products I get, I barely get any bruises. You can just assume they don't form. Haha, even the dermatologist said I have very thick skin), so it's my first surgery. Maybe that's why I wanted to have surgery without implants as much as possible, but what about a fancy double nose? Sleep..? (What is sleep?)
 
They say it won't be fixed if I don't do it, so I don't know how to get it. What are you going to do about using silicone, people like me, who are in trouble the first time? Looking at reviews of ㄹㅂㄹ and ㅅㅇ that I'm interested in, I've seen a few mentions of silicone added, but I haven't seen anyone write mm, so I think it's okay to just not put it in if it's small. Am I thinking wrong?
