Recycle the nasal septum, use ear cartilage, and reinforce it with dermis because the skin on the tip of the nose is thin.
You can remove the silicone, but try touching it. The thinnest silicone is 2mm, and mine is also very thin. It seems to be roughly 2.5mm.
Even if you remove this, the bridge of your nose will be straightened as much as you want. There won't be much difference or it will be subtle. Even if you remove it, you can see the bumps or it may look blue because you made the cut. If you don't mind having a different skin color, I'll take it out. I will go to the operating room and perform the surgery while the patient is awake, so I will take it out and show it to you if I think it looks good. But the probability of that happening is low.
Anyway, since the tip of the nose is set too high, we need to lower it a bit and use the ear cartilage to recycle the nasal septum. Because the skin is thin, dermis reinforcement is necessary.
The tip of the nose looks like a V shape because the nasal septum was already harvested and used, so if you cut out the sagging part on the columella side (I don't remember exactly, I think I had a septum extension surgery anyway, but if you cut out the saggy part on the columellar side), your nose will be unstable and it will collapse. My nose was already long, but unless the person who had the surgery before intentionally made it longer, there doesn't seem to be room to shorten it. But in my case now, my nose looks long, and regardless of what the problem is, the silicone is visible in the area where the bridge of the nose connects to the tip of the nose.
