Is the counseling director’s incentive a gun-buyer? or not

I'm thinking about contouring + nose, but
 I've set a surgery date for the contouring first.
 I'm still thinking about the nose, so I asked the counselor how much it would be to get the nose done.
 
What I'm curious about is whether the counseling director's incentive is gun-buy or n% of the patient's surgery fee!!
 
If I receive an incentive on the cost of my surgery, the counseling director can give me an interest rate if I have another surgery, or
 if I'm worried, he can give me an additional discount.
 
But if it's a gun buy gun, I've already decided to outline it, so I'm wondering if they won't give a discount or contact me often because it's annoying.
