So, I just did it at another hospital that said they would take care of the documents and didn't expect a refund, but since it included the actual cost and the surgery fee, I received a refund equal to the functional cost.
Of course, there may be differences between insurance companies, but the emphasis on absolute is so ridiculous.

I don't know when the terms and conditions will change, but I had surgery by a plastic surgeon at the end of February
and received a good refund. It looks like the surgery went well!
